Job summary

Portsmouth Community Nursing Team is looking to recruit a dynamic, caring, and passionate Health Care Support Worker to join our Community Nursing team to support the provision of high-quality Nursing care to patients within their own homes.

You will work as part of an integrated health and social care team in delivering planned and unscheduled nursing care across the city.

Community Nursing service provides nursing care to patients at home across the 24-hour period with a Day Team (0830-1700) and an Out of hours team (Twilight 1600-00:00, and Nights 2130 0830) seven days a week, 365 days a year.

The Community Localities team provides community nursing interventions for those patients defined as housebound and therefore unable to access such nursing interventions elsewhere. This element is delivered over a 24 hour period with nursing teams organized to deliver care during core hours, twilight hours and night-time hours. The service is provided to all residents with a Portsmouth GP. The Community Nursing Service in Portsmouth is a seven-day service delivered over 24 hours. The Care Home team and Community nursing support service who predominantly care for those patients at the end of their life.

Main duties, tasks & skills required

Working under indirect supervision, as part of the Community Nursing team the post holder will to have responsibility for delivering nursing care to patients within a defined Community Nursing

caseload and within one or more GP practices

Deal empathetically with sensitive information, to include discussing patient's changing condition with patient and/or carer

You will participate in inter-professional working and the development, implementation, and evaluation of the delivery of health care services

To report any changes/concerns to the registered nurse on duty in a timely fashion and record in the patient's records.

Participation in collaboration with other agencies, statutory and voluntary to ensure continuity of care.

To have responsibility of a link nurse role to develop and improve care, building on existing good practice and clinical skills development.

Actively respect patient's privacy and dignity and protect their property and possessions.

As part of the Community Nursing Locality team, assist with the investigations of complaints, participate in the risk management process, critical incident reporting, evaluation, dissemination and change in practice.
